Call with any questions regarding insurance coverage. Co-pay amount is listed on ID card as a "Specialist".   Please bring referral/or a script from PCP or referring doctor. If an ear doctor refers you, you may need referral from PCP. If you are not sure, feel free to ask. All co-pays are due at time of appointment. Lyndhurst Audiology will submit insurance claims. If you do not have insurance, I encourage open and honest discussion of audiology fees. It is the mission of this office to provide quality audiology service to all and to prevent financial constraints from preventing access to audiology services and hearing aid services and assistance.
